There are Nepharim guards on either side of the entrance to this cave. You could sneak past them as suggested in the game script. It is better however to kill both groups before entering. If you are not strong enough to eliminate them, you will certainly not be able to kill the Nepharim Chief and free the prisoners.
The prisoners that Mayor Steele in Cotra asked you to free are locked up in a room in the middle of this cave. The entrance to the fort is well guarded and since the gate is closed you could not storm it from the front. There are however two secret passages which lead into the middle of the fort: one to the east and one to the west. The eastern one will lead you to Renn who will give you a few tips.
As well as the prisoners there are a number of other goodies (part of the Demonslayer and the spell Curing level 3) but you need a fairly strong party to get them.
I usuall take the western secret passage first. After killing some rats you can get into the little guardroom to the west of the main entrance and kill the guards there. This will make your fight easier later on. Then it is is a good idea to take the eastern passage and talk to Renn.

Renn, a soldier from the Cotra Militia who got caught with some others but managed to get away, is hiding in the NE corner. He will tell where the prisoners are being held and give you some useful information about the Nepharim and the fort.
There are a number of different possibilities now. You could go along the passage in the north, through the open portcullis and into the dining hall. I prefer to take the secret passage in the east (south from Renn an then west), thereby getting rid of many of the nepilim in the SE and central areas. This has the advantage of eliminating any possible reinforcements for the party in the dining hall.
If you report back to Renn after freeing the prisoners you will get 1 Rep. and around 35xp.
Before entering the dining hall it is a good idea to prepare yourself for a hard battle. When you get in you wil find the chief Arr'Eh and some of his troops (a nephar warrior and 4 nephars). The Nephars are difficult enough but the chief is really tough. They drop some nice loot including a Platinum Ring and Momentum Gauntlets.
Then you can go east and unlock the door to the prison and free the prisoners. There is another Bronze Key (like the one in the Goblin Caves) in a chest in the SW corner.
In the shrine southeast of the prison cell are two spiritists with some nephar and nephar archers. If you take the dagger (which is actually a Radient Shortblade, worth 1500) from the altar three Cave Demons materialise!
In the middle room in the west are two Nephar mages Herrrm Split-Tail and Vk'Arrr Split-Tail sitting at a table with two nephar warriors. They are tough but when you have disposed of them you can learn Curing L3 from the tome in the corner if your party has a total Arcane Lore of at least 5.
In the NW couner of the dungeon is the crypt Renn warned you about. There you can find two Crypt Demons which are guarding the altar on which the pommel of the Demonslayer is lying.
If you just want to free the prisoners and get your reward from Mayor Steele in Cotra and not bother about the loot here, you could sneak past the two groups of nepharim outside the cave and go straight up to Renn. He hints that there could be another way into the prison cells:
"Into the building just past the gate. The east wall is crumbling. It'd be great if there was another way to get into the cells without being seen, but if there is I haven't found it yet."
Now you can sneak out by the same route and go to Cotra where the mayor will give you your reward.
